The forms below are required by the Salina Soccer Club and/or KYSA for each season (Aug 1st - Jul 31st).

bullet 1st time players joining the Salina Soccer Club will also need to provide a copy of their birth certificate

Guest Player Procedures (Tournaments)
  1. Get medical release and player ‘card’ from players primary team to take with you. (don’t forget to return them)
  2. Go to www.ksysa.org, Tournament & Travel Forms link.
  3. Scroll down to “Kansas on-line e-Travel forms system”
  4. click on purple “Click Here”
  5. Click on the purple “Login to the e-Travel System”
  6. Click on ‘Guest Player”, then Submit
  7. Fill out “Guest Player Permit”, then Submit.
  8. The State office will e-mail you confirmation
  9. Take confirmation and permit information with you
